INTRODUCTION to Essentia Health
Essentia Health is a health care provider with multiple clinics located in North Dakota and Minnesota. We provide express care and occupational medicine services to 10s of thousands of patients/clients each year. We are HIPAA, OSHA and ADA compliant and all personal medical information you provide is protected as a part of our medical records.

Essentia Health has been contracted by your prospective employer obtain medical information and or conduct a pre-placement evaluation, and/or to perform a OSHA Respirator Medical Evaluation (per OSHA 1910.134) and/or render an opinion as allowed by the American with Disabilities Act (ADA).

The Americans with Disabilities Act (ADA), enacted on July 26, 1990 provides comprehensive civil rights protections to persons with disabilities in the areas of employment.

It is VERY important that you provide accurate and complete information to Essentia Health.

This information is being requested as a part of the medical evaluation allowed by OSHA and the ADA. Accurate and complete information is required for successful completion of this inquiry. If you have any questions regarding this please contact your HR representative.
