This form is only for use by DGSOM student leaders to register an event with the SAO and request food for their event. This form is mandatory to request SAO to place orders for SAO and MSC funded events.

You must submit this form a minimum of 5 business days before your event.
All events must be approved and posted in the Medical Student Calendar.
Please note SAO funding can be used August 24 onward- Dec 31 of the 2020-21 academic year, and group holding talks post Dec 31 should apply for food funding to the MSC.
Note: you can apply for MSC funding anytime during the academic year

Must upload a list of attendees a minimum of 5 business days before the event with this template: RSVP Template (Excel Format) This list will be used to order food for your event and submitted to Accounting. $7.50 per person x up to 50 people can be funded for a total of $325.  *This question is required.
