Rhode Island Department of Health: Community Survey


Thank you for taking part in this important survey! This survey is being conducted by the Rhode Island Department of Health and is intended for families and residents who live in Rhode Island. Your feedback will help inform state efforts to set goals and strategies to support the health and wellbeing of Rhode Island families.

How much time will the survey take? The questions should take about 20 minutes to complete.  

Who will see my answers? Your responses will be confidential. The project team from the Rhode Island Department of Health will have access to your answers. Our partners at Abt Associates will also have access to your responses because they are helping to analyze the data. Reports will not include any personally identifying information about the individuals who complete this survey. As with any study, there is a very small risk of breach of confidentiality. However, the study team is highly trained to protect your privacy. 

At the end of the survey there is an optional question asking if you would like to participate in future focus groups. If you wish to do so, please provide your name and contact information. This information will not be shared outside of the study team and will not be included in any reports. 

Do I have to complete this? The survey is voluntary. You are free to skip any questions you feel unable or unwilling to answer. However, we hope you will answer as many questions as possible, since this will improve the quality of the information we collect. If you choose not to complete the survey, this will have no effect on your relationship with the state or your employment.
