Practice Self-Assessment Tool

Thank you for your interest in New York State Patient-Centered Medical Home (NYS PCMH)!

New York State has partnered with the National Committee on Quality Assurance (NCQA) to create NYS PCMH, a PCMH program designed specifically for NYS primary care practices. The Patient-Centered Medical Home (PCMH) model helps primary care practices to provide patient-centered care, improve quality outcomes, and prepare for value-based payment arrangements. NYS PCMH is based on NCQA PCMH 2017 standards.  

Practices eligible to participate in NYS PCMH provide primary care services. This includes pediatrics, geriatrics, internal medicine, and family medicine. Eligible practices can receive Practice Transformation Technical Assistance at no cost to the practice.

This self-assessment tool is intended to be completed by the practice provider lead. However, most questions can easily be answered by a practice/site manager in collaboration with the provider lead.

This self-assessment tool will take approximately 20 minutes to complete.

Please note: completion of this self-assessment does not guarantee your participation in the NYS PCMH program through NYC REACH.  NYC REACH will use the information provided by your practice in this self-assessment to determine if you are eligible to participate in the program and will reach out with next steps.
