Lake County Behavioral Health (LCBH) has partnered with Resource Development Associates (RDA) to assist with preparing the upcoming MHSA 2020-2023 Three-Year Program & Expenditure Plan. The purpose of the MHSA Three-Year Program & Expenditure Plan is to capture and document the community’s vision for services to address mental health issues.
We are conducting this survey across Lake County to understand how well LCBH’s programs, services, and activities meet the community’s mental health needs and what LCBH can do to improve its services. Your responses will be important to inform the MHSA Three-Year Program & Expenditure Plan.
This survey is voluntary and anonymous. This survey is voluntary. You may choose to skip any questions you do not feel comfortable answering. Only RDA will see your response and they will combine your response with others' responses to inform the capacity assessment.
