Dear Colleague,

My name is Dr. Ahmed Alhussain. I am a MSc/Specialty student in the Department of Oral & Maxillofacial Surgery at the Faculty of Dentistry, University of Toronto.

I would like to invite you to participate in my Masters thesis research, which is concerned with the perceptions and practices of Ontario dentists when treating patients receiving bisphosphonates. In particular, we would like to measure the knowledge of Ontario dentists about bisphosphonate-related osteonecrosis of the jaw (BRONJ).

I am inviting you to participate in this research by completing this online questionnaire. Your participation will help us to:

1) measure the awareness of Ontario dentists about BRONJ;
2) assist educators to develop future guidelines and protocols related to BRONJ

The survey will take approximately 7 minutes to complete. There is neither cost nor reimbursement for your participation in this study. You have been selected from the pool of Ontario dentists for this study and your participation is voluntary.

All surveys are confidential, and I will be the only one with access to the data. All necessary measures have been taken to safeguard your privacy. No information that discloses personal identity will be released or printed. You are free to withdraw from the study at any time during the survey by clicking on the "Exit survey" button, and if you withdraw, your withdrawal will not have any effect on your present and future relationship with the Faculty of Dentistry, University of Toronto. It is my intention to publish this survey's findings in scholarly publications, as well as presenting such findings in lectures and professional/academic meetings. Your name will not be used in the reporting of the information in publications or conference presentations.
