Thank you for your interest in our study.  We hope to learn more about eating patterns so that we can develop more effective treatments. We are looking to recruit about 2,000 women aged 18 or older for a study on eating disorders.

This study will involve 3 self-administered surveys over 12 months. The surveys will ask questions about your eating habits, medical history, treatment about eating disorders, and some lifestyle questions. You can refuse any question and the survey is entirely voluntary. You can withdrawal from the study at any time. You can take the surveys online at your convenience.  All you need is access to a laptop or notepad device (e..g, iPad, Kindle Fire, Samsung Galaxy).  You can take the surveys on your web-browser enabled phone, but the questions may be a bit hard to read. 

You will receive $25 for the first survey and $10 each additional survey.  The first survey asks about your history, so it takes about 30-40 minutes to complete. The surveys at 6m and 12m will take about 25 minutes to complete. 

We will only need your name and e-mail address for payment purposes. None of your contact data will be connected to your survey questions because we will assign you a random study ID.  Any data from this study will be only be presented in aggregate form (e.g., percentage of persons).
