Alternative Responses to Mental Health Crisis Survey


The purpose of this survey is to help the City of Minneapolis gather information on people’s experiences with mental health support services during a crisis. Results of this survey will be used to identify areas for improvement for service delivery for the residents and visitors of Minneapolis. 

This is not a reporting tool.  If you have an emergency, please call 911.

This survey is available for anyone to complete and should take around 10 minutes to complete.  Please feel free to be completely honest.  This survey is anonymous and confidential.  Your answers will not have a name or IP address associated with them. 

A Note on Mental Health Hygiene: The City of Minneapolis is grateful for your willingness to speak from your experience, especially during this uncertain time. If you are currently dealing with mental illness, the following questions may trigger symptoms. We recommend that you reduce or alleviate common stressors such as hunger, exhaustion, and lack of hydration before and after.

Some general signs of stress are: rapid heartbeat, tense muscles, feeling overwhelmed, inability to focus, etc.

If symptoms develop during the survey, please feel free to skip questions or leave the survey at any point.
